If you have a qualifying medical condition as evidenced by existing medical records from your Primary Care Physician (PCP) then we will help you to get your Medical Use of Marijuana (MUM) certificate.

Qualifying Conditions include the following:
Cancer; HIV/AIDS; Glaucoma. Or, a disease, or treatment of that disease, which causes: Severe Pain; Severe Nausea; Cachexia (Wasting Syndrome); Severe Spasms; Seizures. Asthma qualifies under the Severe Spasm category, though it is wise not to smoke marijuana for the treatment of asthma; rather, it is best to use a vaporizer, such that there is no smoke, soot, or other harmful material present in the vaporized medicine.

You must have one or more of these conditions to qualify, and medical history proving these diagnoses must be present in your existing medical record in order for you to participate in the program. These medical records proving your condition must not be more than one year old and must be made by a Medical Doctor with the date of service and doctor's signature.
Certification Process: Submit your medical records to us; either send them directly to us if you have them with you, or we will get your consent to get them from your doctor. Then, we will review your medical records to be sure that you do indeed qualify for the Medical Use of Marijuana. Once that is done you will register as our patient and we will help you to fill out the State of Hawaii forms from the Narcotics Enforcement Division. Our Medical Doctor will see you as a patient, and if he believes that you would benefit from the Medical Use of Marijuana then he will certify your condition and submit your application to the State of Hawaii. Application for our services does not constitute an automatic certification for the State Medical Marijuana program. Once the correctly completed application is processed by the State of Hawaii and your Certificate is issued, you may grow your own marijuana, or your designated caregiver can grow your marijuana for you. You must repeat this every year.
